Job Description:
A company is looking for an RN Care Manager.
Key Responsibilities
Provide exceptional care management services to designated patient
populations
Engage with multidisciplinary teams to develop effective business
solutions
Assess patient needs and deliver high-quality care solutions to
improve health outcomes
Required Qualifications
Associate's Degree in Nursing required; BSN required within 5 years
for new ASN RNs
Active Registered Nurse (RN) license in Indiana or Nurse Licensure
Compact (NLC) RN license
Certification in Care Management preferred
3-5 years of relevant experience required
Knowledge of Medicare/Medicaid, insurance, and regulatory
guidelines preferred
