Cumming services patients throughout Cherokee, Forsyth, Nort Fulton, and Gwinnett County and is seeking LPNs for full time or prn schedules. Pick your work schedule and service area!

  • Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
  • Complete all documentation and records in a timely and accurate manner
  • Follow established policies and procedures as related to the scope of practice
  • Maintain continuity of patient care in a home care setting LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
